It's not difficult at all.
Update: First, you need to create the tabs in the first place. I use Static HTML.

- First you'll need to create an image. A good size is 111px wide by 74 px wide. (If you're a former Picnik user, there are some alternative image editors listed here.)
- When you're ready to install it, click the down arrow at the far right of the row of visible tabs (mine is just to the right of my Likes).
- One you do, you'll have the option to edit your tabs. Hover over the tab you want to edit and click the pencil icon that appears in the top right corner. Select "Edit Settings" from the menu.
- Click "Change" next to Custom Tab Image" and follow the prompts to upload.
How to Make a Blog Button provides some basic tips for creating your own image. Note that I am using Picnik in the tutorial which is no longer available, but I did another tutorial in A Free Alternative to Picnik: Pixlr Express. It works very similarly.
